Your First Visit

A physical therapy evaluation is required for new patients and sometimes returning patients with new injuries. You should expect your first visit to last for about 1 hour. It is important to wear clothing that allows you to comfortably stretch and exercise. We have rooms at our facility available so you can change into proper clothing when you arrive.

There are several documents that we require before your first visit. These documents will be completed online. If they are not completed before your first visit, please arrive 15 minutes early to allow time to complete them. We thank you for your cooperation.
