Your First Visit
A physical therapy evaluation is required for new patients and sometimes returning patients with new injuries. You should expect your first visit to last for about 1 hour. It is important to wear clothing that allows you to comfortably stretch and exercise. We have rooms at our facility available so you can change into proper clothing when you arrive.

There are several documents that we require before your first visit. Please see the below list for required documents. Due to the time needed to complete paperwork, please arrive 15 minutes early if you have not yet completed your paperwork.
Private Insurance (Call to hear the Insurances CPR Accepts)
- Photo Identification (driver's license, ID card, etc.)
- CPR Patient Intake Form (PDF) or Spanish Patient Intake Form (PDF)
- Insurance Cards
- If required by insurance company
- Referral from your doctor
Medicare and Medicaid

Worker's Compensation (ex. work related injury)

- Adjuster's Name and Contact Information

Self Pay
